The CDRC Pneumococcal Vaccination Resource Suite supports GP practices to systematically identify, assess and manage patients who may be eligible for pneumococcal vaccination. The resources help practices improve uptake while reducing the administrative burden associated with case finding, patient review and follow-up.
The resource suite includes:
- Reports for population-level case finding, prioritisation, performance monitoring and data-quality review.
- A clinical template to support individual patient assessment, recording and vaccination.
- Patient Status Alerts to identify potentially eligible patients opportunistically when the patient record is opened.
Together, these resources provide a structured pathway from population-level identification to individual patient review, clinical action and follow-up.
Important: The CDRC resources support clinical review but do not replace clinical judgement. Eligibility and the appropriate vaccination must be confirmed by the responsible clinician using current national and local guidance.
Accessing the CDRC Resources
To access the below resources you will need to be a member of the DCS group on SystmOne. To do this, please follow the instructions on the CDRC SystmOne Access webpage.
Pneumococcal Reports
The CDRC Pneumococcal Vaccination reports identify patients who may be eligible for vaccination using coded information held in SystmOne.
The searches take account of:
- relevant clinical guidance;
- the Enhanced Service specification;
- applicable pneumococcal business rules;
- vaccination and eligibility information recorded in the patient record.
These sources do not always define cohorts in exactly the same way. A patient may therefore be clinically eligible without being identified automatically by the business rules.
CDRC reports support case finding and prioritisation. The responsible clinician must confirm that the patient is eligible and that vaccination is clinically appropriate before administration.
Adding a patient to the business-rule cohort
If an eligible patient has not been identified by the CDRC searches, the code Requires a pneumococcal vaccination (XaM2n) can be added to the patient record.
Only add this code after confirming that the patient meets one of the current eligibility categories. The pneumococcal vaccination template can also prompt the user to add the code as part of the vaccination workflow.
Finding the reports in SystmOne
The reports are located within: Reporting > Clinical Reporting > CDRC Vaccinations > Pneumococcal
The reports are separated into four folders:
- Pneumococcal 1 Cohorts
- Provides an overview of the practice pneumococcal vaccination population, including patients with and without a recorded vaccination.
- Pneumococcal 2 Eligible Patients
- Identifies patients who may currently require pneumococcal vaccination.
- Patients identified by these reports must be clinically reviewed before invitation or vaccination.
- Pneumococcal 4 Performance
- Provides performance reports to support:
- vaccination uptake monitoring;
- practice-level performance review;
- CQRS or contractual reconciliation, where applicable;
- identification of gaps in recorded achievement.
- Provides performance reports to support:
- Pneumococcal 5 QC and Admin
- Provides quality-control and administrative reports that identify records requiring review.
Quality-Control and Admin Reports
The following reports beginning support coding and data-quality improvement.
For each patient:
- Review the patient’s clinical eligibility.
- Check whether relevant diagnoses or risk factors have been coded correctly.
- Confirm whether the patient was vaccinated under clinical guidance, specialist advice or another valid indication.
- Add or correct clinical codes only where supported by the patient record.
- Provide feedback to the administering clinician if the record indicates that further review is required.
Run these reports regularly and before the end of the relevant reporting period.
! Quality Reports will support quality-improvement activities:
Vaccinated but no expected vaccination code
This report identifies patients with evidence of a recent pneumococcal vaccination but without the expected vaccination code.
Review the clinical record before making any changes. If the vaccination was administered by your organisation and the record supports the change, add the appropriate pneumococcal vaccination code in accordance with your local coding policy.
Do not add a code solely to remove a patient from the report.
Vaccinated but not included in the business rules
This report identifies patients with a recorded pneumococcal vaccination who do not appear to meet the coded Enhanced Service or business-rule criteria.
Pneumococcal Patient Status Alert
CDRC Patient Status Alerts are not activated automatically when an organisation joins the DCS Organisation Group. This allows each organisation to retain control over its local SystmOne configuration.
To request activation:
- Email contact-cdrc@healthinnovationnenc.org.uk.
- Request activation of the CDRC Vaccination and Immunisation Patient Status Alert.
- A member of the CDRC team will support the organisation through the activation process.
Once activated, the pneumococcal alert appears beneath the patient information area when an eligible patient’s record is retrieved.
Selecting the alert opens the Pneumococcal Vaccination – CDRC Template.
Pneumococcal Vaccination Template
The Pneumococcal Vaccination – CDRC Template brings together information to support an individual patient review.
Depending on the information available in the patient record, the template may display:
- the reason the patient appears eligible;
- persistent or expiring vaccination exceptions;
- previous vaccinations;
- relevant pneumococcal vaccination history;
- current PSD information;
- clinically relevant observations;
- prompts relating to coding or business-rule inclusion.
The controls on the left-hand side of the template allow users to:
- record a vaccination invitation;
- record a refusal or decline;
- review vaccination exceptions;
- access the appropriate vaccine-administration template;
- record the vaccination and associated clinical information.
Before recording or administering a vaccine, confirm:
- the patient’s current eligibility;
- previous pneumococcal vaccination history;
- any contraindications, exceptions or postponements;
- the correct vaccine product and schedule;
- the appropriate authorisation mechanism;
- that the clinical record accurately supports any eligibility code added.
