The CDRC Family/Household Cause for Concern resource helps practices identify patients with safeguarding concerns who have close contact with vulnerable people, such as children or adults at risk.
The resource supports users to consider the needs and safety of vulnerable household or family members when reviewing the index patient. It brings together:
- A clinical report providing a structured view of the practice register.
- A patient status alert highlighting recorded family or household concerns.
- A dedicated template for reviewing and recording relevant safeguarding information.
- Information about relatives, carers and professionals.
- Tools for updating the patient’s register status.
- Access to supporting domestic abuse, MARAC, police domestic incident and FGM resources.
The register may include, for example, a patient with significant drug or alcohol problems living with children, a patient who presents a risk to others living with a vulnerable adult, or a patient with relevant historical safeguarding information who is pregnant.
Accessing CDRC Resources on SystmOne
To use these resources in SystmOne, you will need access to the CDRC resources through the DCS Organisation Group.
If you do not currently have access, please follow the instructions on the CDRC SystmOne Access webpage.
Once access has been enabled, the reports can be found in the relevant CDRC folders and the templates can be opened from within the patient record.
Clinical Reports
The CDRC Family/Household Cause for Concern clinical report is included within the Safeguarding Register section.
Reports can be accessed in SystmOne Clinical Reporting from: CDRC > Safeguarding
1. Safeguarding Register
The Family/Household Cause for Concern Register report identifies patients with recorded safeguarding concerns who may have close contact with another vulnerable person, such as a child or an adult at risk.
Patients are included in the register through relevant coded information indicating a family or household risk, without a subsequent code indicating that the family is no longer a cause for concern. Children already included in the practice’s Safeguarding Children Register are excluded from this register.
Patient Status Alerts
CDRC Patient Status Alerts are not activated automatically when an organisation joins the DCS Organisation Group. This allows each organisation to retain control over its local SystmOne configuration.
To request activation:
- Email contact-cdrc@healthinnovationnenc.org.uk
- Request activation of the CDRC Safeguarding Patient Status Alert.
- A member of the CDRC team will support the organisation through the activation process.
Once activated, the Safeguarding PSA appears to make relevant safeguarding information more visible when a child’s record is accessed.
Patients included in the At Risk of Harming Others Register receive an alert. Additional alerting is used where a patient is recorded as presenting a high risk
When the relevant patient’s record is retrieved, once of the aforementioned PSAs will be shown. Clicking on the icon will direct you to the Safeguarding Family is Cause for Concern CDRC Template as shown below.

The Safeguarding Family is Cause for Concern CDRC Template rings relevant safeguarding and household information together in one structured view. It helps users review the patient’s register status, record summaries and discussions, view key relationships and professionals, and access supporting safeguarding tools
How to Access
With the patient record open, use the search bar in the lower left-hand corner and search for: Safeguarding Family is Cause for Concern CDRC
Alternatively, press F12, search for and select the template from the search results.
What the Template Includes
The template includes dedicated pages for:
- Family/Household Cause for Concern: Provides an overview of the patient’s relevant safeguarding information, including register status, safeguarding summary and previous discussions. The page also provides access to relevant coded information and supporting safeguarding tools. [cdrc.nhs.uk]
- Relatives and Carers: Displays relatives, carers and other relationships recorded in SystmOne, helping users review the people associated with the patient and identify vulnerable family or household members. [cdrc.nhs.uk]
- Professionals: Displays professionals and services recorded as being involved with the patient, supporting coordinated review and communication where appropriate
The main template page also provides access to information and tools relating to:
- Domestic abuse.
- Police domestic incident reports.
- MARAC information.

- Other coded safeguarding information, including missed appointments and out-of-hours activity
