The CDRC Precision Monitoring Template will intelligently display pages for DMARDs and High Risk Drugs that require monitoring. The individual pages will alert clinicians to the monitoring responsibilities, either primary or secondary care and enable this to be coded. If Primary Care monitoring is coded the required monitoring for these drugs will be included in the Precision Monitoring Template. The template will also alert clinicians whether a patient is considered a high risk patient, requiring more frequent monitoring and whether there are any abnormal results for monitoring, requiring action.

This resource can be used in conjunction with the CDRC Quality Population reporting resources.
The below screenshot for the DMARD Leflunomide shows that the monitoring requirements are unclear. Using the template, it can be accurately coded to enable appropriate monitoring to be arranged in Primary Care. The template can also be updated to transfer care back to Secondary Care if needed.
There are alerts displayed in RED if a patient is a high risk patient, or if there are any abnormalities in the monitoring that has taken place.
