Safeguarding At Risk of Harming Others

At Risk of Harming Others Register

Patients are included in the organisation’s at risk of harming others register via the presence of a Read code indicating they are a risk WITHOUT a subsequent code of At low risk of harming others. 

Read codes that indicate possible risk include: 

  • Perpetrator of domestic abuse 
  • Perpetrator of physical, sexual, emotional abuse 
  • Violent patient register 
  • On the sex offenders register 
  • At risk of harming others codes

How to Access

In the lower left hand corner use the search bar, type in ‘At Risk of Harming Others CDRC’ and select the following template:

Alternatively, press F12 and search for ‘At Risk of Harming Others CDRC’, this will open the aforementioned template.

The Risk of Harming Others template provides an overview for the selected patient. A thumbnail sketch can be entered at the top of the page to provide an aide-memoir for meetings etc. This is added by clicking the Safeguarding Summary button or right clicking the current thumbnail and choosing Copy Questionnaire.
