The CDRC At Risk of Harming Others resource supports GP practices to identify, record and review information about patients who may present a risk to other people.
The resource brings together:
- Clinical reports providing structured views of the practice register.
- Patient status alerts highlighting recorded risk information.
- A dedicated template for reviewing and recording relevant information.
- Tools for recording risk levels and updating register status.
- Information about relevant household members and involved professionals.
- Access to supporting MARAC, MAPPA and police domestic incident recording tools.
Accessing CDRC Resources on SystmOne
To use these resources in SystmOne, you will need access to the CDRC resources through the DCS Organisation Group.
If you do not currently have access, please follow the instructions on the CDRC SystmOne Access webpage.
Once access has been enabled, the reports can be found in the relevant CDRC folders and the templates can be opened from within the patient record.
Clinical Reports
The CDRC At Risk of Harming Others clinical reports are organised into two relevant groups:
- Safeguarding Register
- Safeguarding Registration and Deduction
The reports can be accessed in SystmOne Clinical Reporting from: CDRC > Safeguarding
1. Safeguarding Register
The Safeguarding Register reports provide structured views of patients with information indicating that they may present a risk of harming other people.
The resource helps practices maintain a structured register, make relevant information more visible and support consistent review and recording.
Register inclusion is based on relevant coded information recorded in SystmOne. This may include information relating to domestic abuse, other forms of abuse, violent patient records, sex offender register information or specific risk-of-harming-others codes. [cdrc.nhs.uk]
A patient may remain on the register until an appropriate subsequent code indicating that the patient is at low risk of harming others is recorded.
4. Safeguarding Registration and Deduction
The Safeguarding Registration and Deduction reports identify patients on the At Risk of Harming Others Register who have recently joined or left the practice.
Patient Status Alerts
CDRC Patient Status Alerts are not activated automatically when an organisation joins the DCS Organisation Group. This allows each organisation to retain control over its local SystmOne configuration.
To request activation:
- Email contact-cdrc@healthinnovationnenc.org.uk
- Request activation of the CDRC Safeguarding Patient Status Alert.
- A member of the CDRC team will support the organisation through the activation process.
Once activated, the Safeguarding PSA appears to make relevant safeguarding information more visible when a child’s record is accessed.
Patients included in the At Risk of Harming Others Register receive an alert. Additional alerting is used where a patient is recorded as presenting a high risk
When the relevant patient’s record is retrieved, once of the aforementioned PSAs will be shown. Clicking on the icon will direct you to the At Risk of Harming Others CDRC Template as shown below.
At Risk of Harming Others Template
The At Risk of Harming Others CDRC Template brings relevant risk and safeguarding information together in one structured view. It helps users review the patient’s register status and recorded risk information, record summaries and discussions, view key relationships and professionals, and access supporting MARAC, MAPPA and domestic incident recording tools.
How to Access
With the patient record open, use the search bar in the lower left-hand corner and search for: At Risk of Harming Others CDRC
Alternatively, press F12, search for and select the template from the search results.
What the Template Includes
The template includes dedicated pages for:
- Risk of Harming Others: Provides an overview of the patient’s recorded risk and safeguarding information, including register status, safeguarding summary and previous discussions. The page also provides access to relevant coded information and supporting recording tools.
- Relatives and Carers: Displays relatives, carers and other relationships recorded in SystmOne, helping users review people associated with the patient and identify other potentially vulnerable people within the household.
- Professionals: Displays professionals and services recorded as being involved with the patient, supporting coordinated review and communication where appropriate.
- At Risk of Harming Others Register: Provides structured functions for adding a patient to, or removing a patient from, the practice register and recording the relevant level of risk.
The main template page also provides access to information and tools concerning:
- Other people of concern within the household, including children or vulnerable adults.
- MARAC information.
- MAPPA information.
- Sex offender register information.
- Police domestic incident reports.
- Other coded safeguarding information, including missed appointments and out-of-hours activity.
Clicking on specific quick action buttons, located on the left-hand side of the template, will open the relevant template.
For example, clicking on Sec Offenders Register will open the appropriate template allowing you to input further information.
