Lifestyle change interventions are critical to preventative healthcare. 

Information about lifestyle change referrals is integrated into CDRC to try to maximise uptake of these interventions. 

Templates where lifestyle change might be appropriate, show the most recent information about referrals or where are referral has been declined.  The example below is from the Core Data Entry template. 

Clicking on the Lifestyle Referral button will open the Lifestyle Programme template, showing the relevant information for the local area along with weblinks, referral proformas, information leaflets where available.

Ways To Wellness www.waystowellness.org.uk 
Ways to Wellness is for people with certain long-term health conditions who attend 
GP practices within what was Newcastle West Clinical Commissioning Group 
area The service provides patients with a Link Worker to meet one-to-one with 
them to identity and work to overcome their current barriers to managing their 
long-term conditions. The eligible long-term health conditions are: 
Chronic breathing difficulties (CORD) or Asthma 
Diabetes Type 1 or Type 2) 
Hean Disease 
Epilepsy 
Thinning of the bones (osteoporosis) 
Any of the above with depression and/or anxiety
