The CDRC 6-8 Week Postnatal Check template addresses the assessment and management of patients to support their postnatal care. There are dedicated pages for Physical Health, Mental Health, Social History, Examination and Investigations & Follow up.
How to Access
To access this template, your unit will need to be a member of the DCS group on SystmOne. To do this, please follow the instructions on the CDRC SystmOne Access webpage.
Once you have access, or are already a member of this group, with a patient retrieved type in “6-8 Week Postnatal Check – CDRC” into the search bar located in the lower left-hand corner of the main screen and select the following template:
Alternatively, press F12 and search for “6-8 Week Postnatal Check – CDRC” and this will return the aforementioned template.
6-8 Week Postnatal Check Template
Home Page
The home page provides a Postnatal Review of the patient through information from the Postnatal Check Questionnaire.
You can send the Postnatal Check Questionnaire to the patient prior to their appointment by using the Communications Annexe.
With the patient’s record retrieved, open the Communications Annexe and click ‘Select Questionnaire’:
Search for ‘postnatal’ and select the following questionnaire. You can then enter message text in your Email or SMS to this patient asking them to complete the questionnaire prior to their appointment.
If the patient has not completed the Postnatal Check Questionnaire prior to their appointment, clicking on the Postnatal Check Questionnaire box will open this questionnaire for completion.
Clicking on the Information Resources box will open up additional resources which are of benefit for Postnatal care.
Physical Health Page
Provides a brief overview of the physical health of the patient while allowing you to input relevant patient information.
Postnatal Mental Health Page
Provides a brief overview of the mental health of the patient while allowing you to input relevant patient information.
Social History Page
Provides a brief overview of the social history of the patient while allowing you to input relevant patient information, links to websites that provide further information are also accessible via this page.
Examination Page
The Examination page allows you to input notes from performed examinations along with BP, weight, height and BMI data.
Patient information on Weight Management and Exercise can be provided from this page. You can also conduct Weight Management and Exercise Referrals.
Clicking on the Weight Detail and Exercise Detail box will open up separate templates which shows more detailed information.
Investigations & Follow up Page
