NHS England has recently included the ‘H/O: dementia’ code into the dementia QoF code cluster, which traditionally has not been used in primary care to denote a diagnosis of dementia. This addition appears to have increased practices’ Dementia Register by approximately 6%.
From an initial audit, many of these ‘new patients’ do not have dementia and are now inappropriately added onto the dementia register. This code may need to be marked in error (or often, a request sent to the community teams request them to mark it in error).
The CDRC team has developed a search, highlighted below, to identify these ‘new patients’ so that you can assess the validity of their diagnosis.
