Safeguarding Family/Household Cause for Concern

Family/Household Case for Concern Register

This register identifies patients with safeguarding concerns (victims or perpetrators) who have close contact with other vulnerable people such as children or vulnerable adults. Examples might include a parent with significant drug and alcohol problems who lives with their children; a violent patient who lives with someone with a significant learning difficulty; a pregnant woman with a past history as a perpetrator of child abuse 

Effective flagging of these patients can help to ensure the needs of the vulnerable person are considered when seeing the index patient. 

Patients are included in the organisation’s Family/Household which is Cause for Concern register via the presence of a Read code indicating there is a risk e.g. Family is cause for concern, unborn child is cause for safety concern WITHOUT a subsequent code of Family no longer cause for concern. 

Children who are on the practice’s child safeguarding register are not counted in this register.

They will also often have an icon to indicate they, themselves, are vulnerable or at risk of harming others. 

Family/Household Cause for Concern Template

How to Access

In the lower left hand corner use the search bar, type in ‘Safeguarding Family is Cause for Concern CDRC’ and select the following template:

Alternatively, press F12 and search for ‘Safeguarding Family is Cause for Concern CDRC’, this will open the aforementioned template.

The Family/Household Cause for Concern Template provides an overview of safeguarding for the selected patient. A thumbnail sketch can be entered at the top of the page to provide an aide-memoir for meetings etc. This is added by clicking the Safeguarding Summary button or right clicking the current thumbnail and choosing Copy Questionnaire. 

The Discussion box is used to record important narrative. Previous discussions can be followed down the right hand side of the template. 

The Show All Safeguarding Information button opens a template that shows all coded information that might be relevant to safeguarding, including out of hours attendance and DNAs.
