Digital Weight Management

These reports can be used to identify patients to consider for invitation to the NHS Digital Weight Management Programme. This programme supports adults living with obesity who also have a diagnosis of diabetes, hypertension or both, to manage their weight and improve their health.
Obesity

These reports can be used to identify your patients who are obese and/ or who are overweight.
GLP1 NHS Obesity Programme

National Institute for Health and Care Excellence (NICE) recommends Tirzepatide (Mounjaro®) for the management of obesity for certain patients. Tirzepatide is a novel dual GIP/GLP-1 receptor agonist recommended for managing obesity alongside a reduced calorie diet and increased physical activity in adults.
These reports will support the identification of patients who are potentially eligible for Tirzepatide, but a full review of the patient record is required. CDRC have developed a visualisation to support this process. See Visualisation below.
Tirzepatide Claims
The following reports located in the CDRC Contracting > Weight Management are required for the Q2 & Q3 Obesity Medication Pathway Claims. You will be asked to submit a claim form, sent from the NENC ICB, based on the activity for each stage:
- Stage 1: Patient initiation payment
- Stage 2: First six monthly monitoring complete (or discontinued before 6th review)
- Stage 3: All monitoring completed (or discontinued before 12th review)
- Stage 4: Discontinuation/Offboarding payment
It isn’t possible to get the Stage 3 payment in October, as it is not possible to conduct 5 monthly reviews and then offboard a patient by the end of September (maximum would be an initial review followed by 3 monthly reviews).

Visualisation
Visualisations are accessed using the search bar in the bottom left-hand corner of SystmOne’s main screen.
GLP1 NHS Obesity Programme
The “GLP1 Mounjaro Weight Loss Programme – CDRC” visualisation helps the identification process for whether patients are eligible for Tirzepatide (Mounjaro®); patient’s may meet the criteria for intervention however certain tests may have to be taking before initiating this medication.
The first section highlights current likely eligibility information, there is a drop down button which provides further information.

This patient meets the BMI criteria and is hypertensive on treatment. However, this patient has overdue HbA1c and overdue Lipids – so they may become eligible if those checks were done. This screen also identified that this patient has sleep apnoea but does not have a CPAP code.

Once the patients meets the criteria for this programme, you can expand the Obesity Medication Initiation dropdown to show recent blood tests and whether you need to update them. You can also use the buttons on the left-hand side of the screen to complete the GLP1 Eligibility Checklist & the GLP1 Patient Checklist, which runs through all the questions needed for the patient.

The Review section has button to collect Blood Pressure and BMI, weight loss calculator and then review template which asks all the relevant questions (with relevant sections greyed out if not applicable e.g. doesn’t have diabetes).
