SystmOne Respiratory Resources

Introduction

CDRC respiratory resources support GP practices with the identification, assessment, review and ongoing management of patients with Asthma, COPD and other respiratory conditions.

The resource suite includes:

  • Population Reports to support Quality Improvement
  • Respiratory Disease Visualisation
  • Asthma Clinical Template
  • COPD Clinical Templates
  • Patient Status Alerts and Automatic Protocols

Accessing CDRC resources on SystmOne

To access the below resources you will need to be a member of the DCS group on SystmOne. To do this, please follow the instructions on the CDRC SystmOne Access webpage.

Respiratory Population Reports

These report help practices identify patients who may require review, investigation or optimisation of treatment.

The following reports are all located within the folder CDRC Quality > Respiratory

To improve the identification of the relevant reports, there are sub-headers within the folder structure to make it easier to identify the reports you would like to run,

The Asthma reports identify patients coded with Asthma who may require further assessment, review, or action.

The Asthma Reports, identifies your existing Asthma patients who require specific actions and/or management. Such as SABA Overuse identifies patients who are potentially overusing their SABA inhaler, you may with to invite these patients in for a review.

The Concordance Alerts reports are available to identify patients with potential poor inhaler concordance.

The COPD Reports, identifies your existing COPD patients who require specific actions and/or management.

The Inhalers reports help to identify patients who may require intervention or may benefit from switching to a more environmentally friendly alternative.

Respiratory Disease Optimisation Visualisation

The Respiratory Disease Optimisation – CDRC visualisation brings together relevant respiratory information in one place, helping clinicians review the patient’s history, medication and current management.

To open the visualisation:

  1. Search for and open Launch Patient Visualisation.
  2. Search for Respiratory Disease Optimisation – CDRC.
  3. Select the visualisation from the returned results.

The information presented will vary according to the patient’s diagnosed conditions, medication, lifestyle information and coded clinical history.

The grey boxes are quick-action buttons designed to support efficient patient management. They allow you to record information directly in the patient record or access relevant guidance and additional resources.

If you wish to conduct a COPD Review from this visualisation, click on ‘Review‘

if the patient is eligible for an OPTIMISE review, the following protocol and alert will activate:

Clicking on ‘Yes’ will add the relevant code to the patient record, signifying you are going to conduct a detailed COPD Review.

Asthma Clinical Template

The Asthma – CDRC template provides a structured overview of the patient’s asthma management and supports consistent assessment, coding and care planning.

You can open this template via the visualisation shown above, or by:

  1. Press F12 or use the Search Bar located in the bottom left-hand corner of the screen.
  2. Search for Asthma – CDRC.
  3. Select the template from the results.

The template includes pages covering:

  • Current asthma management
  • Exacerbation Assessment
  • Personalised asthma action plans
  • Diagnostic information
  • Risk of exacerbation
  • QOF requirements and personalised care adjustments

The Exacerbation page allows for the accurate recording of key information required during assessment.

The Action Plan page allows the viewing, creation and updating of a personal asthma plan.

The Diagnosis page shows the key information required to diagnose asthma.  This includes: possible triggers, personal and FHx of atopy, allergy and anaphylaxis; spirometry; FeNO; PEFR variability; asthma challenge results.

The Risk of Exacerbation and Exceptions page provide key factors that would increase the risk of an exacerbation in this patient while also being able to accurately record key information

COPD Review Template

The COPD Review – CDRC template supports structured COPD reviews and the consistent recording of relevant clinical information.

You can open this template via the method shown above, or by:

  1. Press F12 or use the search-bar.
  2. Search for COPD Review CDRC.
  3. Select the template from the results.
  4. Alternatively, you can use the visualisation above to open this template.

The template supports the recording and review of information including:

  • Previous COPD review details
  • CAT and MRC scores
  • Current symptoms and management
  • Mental health and wellbeing
  • COPD action plans
  • Recalls and personalised care adjustments

The Review 1 page shows when the latest review was conducted along with a summary of previous key information such as the patient’s latest CAT & MRC Score. The clinician can efficiently input information to conduct a structured review.

The Review 2 page places focus on the patient’s mental health, Action Plans, allow the user to add any QoF Exceptions, and to add a recall if required.

Opt-in Resources – Patient Status Alerts and Protocols

CDRC Patient Status Alerts are not activated automatically when an organisation joins the DCS Organisation Group. This allows each organisation to retain control over its local SystmOne configuration.

To request activation:

  1. Email contact-cdrc@healthinnovationnenc.org.uk.
  2. Request activation of the CDRC Respiratory Patient Status Alerts and Protocols.
  3. A member of the CDRC team will support the organisation through the activation process.

Once activated, the following alerts appears beneath the patient information area when an eligible patient’s record is retrieved:

Automatic Protocols to improve coding of asthma exacerbations

The coding requirements for asthma are increasingly complex. The following protocols will help keep on top of this.

This protocol will trigger for patients with asthma who are prescribed oral steroids if they have not had a code for exacerbation of asthma recorded during the same consultation.

Machine generated alternative text:
Question
You are prescribing steroids to this patient with asthma.
Do you wish to record 'acute exacerbation of asthma'?
This automatic alert is part of the CDRC Asthma Resources.
You can disable it if is not wanted - Disabling Protocols
The protocol is called Asmma/ COPD EncerDaüon Coding AUTO •

If the patient has COPD and asthma you will be offered the option of coding exacerbation of asthma or COPD or both.

Whenever exacerbation of asthma is coded a prompt will be displayed to advice the user to arrange a follow up within a few days.

Warning if a new diagnosis of asthma is recorded and the QoF business rules for recording spirometry, peak flow and FeNO have not been satisfied.
