Respiratory Resources

The following resources are available to support you and your practice with the management of your patients with Respiratory Conditions

Accessing CDRC resources on SystmOne

To access the below resources you will need to be a member of the DCS group on SystmOne. To do this, please follow the instructions on the CDRC SystmOne Access webpage.

Respiratory Quality Improvement Reports

There are multiple reports to support the identification, management and optimisation of treatment for respiratory conditions such as Asthma and COPD. There are sub-headers within this folder to make it easier to identify the reports you would like to run.

These reports are in the folder CDRC Quality > Respiratory

The Asthma Reports, identifies your existing Asthma patients who require specific actions and/or management. Such as SABA Overuse identifies patients who are potentially overusing their SABA inhaler, you may with to invite these patients in for a review.

The Concordance Alerts reports are available to identify patients with potential poor inhaler concordance.

The COPD Reports, identifies your existing COPD patients who require specific actions and/or management. For the OPTIMISE & PREDICT Projects report ? COPD 3.1 Higher Risk Patients – May Benefit from Intervention identifies your high-risk COPD patient cohorts who require more frequent reviews and potential optimisation of their COPD.

The Inhalers reports help to identify patients who may require intervention or may benefit from switching to a more environmentally friendly alternative.

Asthma CDRC Template

The Asthma CDRC template has been designed to standardise consultations, providing an overview of the patient’s current Asthma management and allow for the accurate coding of data.

How to Access

In the lower left hand corner use the search bar, type in ‘Asthma – CDRC’ and select the following template:

Alternatively, press F12 and search for ‘Asthma – CDRC’, this will return the aforementioned template.

The Asthma page provides an summary of the patient’s current Asthma management, this template should be completed after the completing the Core Data template where smoking, weight etc., is covered and recorded.  The items in a red background are required for QoF.

The Exacerbation page allows for the accurate recording of key information required during assessment. 

The Action Plan page allows the viewing, creation and updating of a personal asthma plan. 

The Diagnosis page shows the key information required to diagnose asthma.  This includes: possible triggers, personal and FHx of atopy, allergy and anaphylaxis; spirometry; FeNO; PEFR variability; asthma challenge results. 

The Risk of Exacerbation and Exceptions page provide key factors that would increase the risk of an exacerbation in this patient while also being able to accurately record key information

COPD Review CDRC Template

The COPD Review CDRC template has been designed to standardise COPD Reviews, providing an overview of the patient’s current COPD management and allow for the accurate coding of data.

How to Access

In the lower left hand corner use the search bar, type in ‘COPD Review CDRC’ and select the following template:

Alternatively, press F12 and search for ‘COPD Review CDRC’, this will return the aforementioned template.

The Review 1 page shows when the latest review was conducted along with a summary of previous key information such as the patient’s latest CAT & MRC Score. The clinician can efficiently input information to conduct a structured review.

The Review 2 page places focus on the patient’s mental health, Action Plans, allow the user to add any QoF Exceptions, and to add a recall if required.

Respiratory Disease Optimisation – CDRC Visualisation

The Respiratory Disease Optimisation – CDRC Visualisation has been designed to standardise the optimisation and management of patients with Respiratory Diseases.

How to Access

In the lower left hand corner use the search bar, type in ‘launch patient visualisation’ and select the following template:

Then type ‘Respiratory Disease Optimisation – CDRC’ into the search bar, and select the following result:

This will open the visualisation

The Respiratory Disease Optimisation – CDRC visualisation provides a summary of relevant patient information, allows for accurate coding, prescription and review of medications, provision of advice and guidance and the ability to conduct a COPD Review.

The information shown will differ patient to patient based on condition, lifestyle, medication and coded history.

To conduct a COPD Review click on the Review button next to QoF (which signifies that this patient’s COPD code is aligned to QoF).

Opt-in Resources

The coding requirements for asthma are increasingly complex (and at times nonsensical).  The following resources will help keep on top of this.  As they are a little intrusive, these protocols need to be enabled at individual practices or ICBs. Contact contact-cdrc@healthinnovationnenc.org.uk to enable them

Improve coding of asthma exacerbations 

This protocol will trigger for patients with asthma who are prescribed oral steroids if they have not had a code for exacerbation of asthma recorded during the same consultation.

The protocol is called Asmma/ COPD EncerDaüon Coding AUTO •

If the patient has COPD and asthma you will be offered the option of coding exacerbation of asthma or COPD or both. 

Whenever exacerbation of asthma is coded a prompt will be displayed to advice the user to arrange a follow up within a few days. 

Warning if a new diagnosis of asthma is recorded and the QoF business rules for recording spirometry, peak flow and FeNO have not been satisfied.
