he CDRC NHS Health Checks resource suite provides practices with tools to:
- Identify patients eligible for an NHS Health Check.
- Monitor programme performance.
- Target specific patient cohorts.
- Support delivery of the NHS Health Check consultation.
- Identify participants within the Healthy Heart Checks Programme.
- Display patient-level status alerts to support opportunistic interventions.
Accessing CDRC Resources on SystmOne
To use these resources in SystmOne, you will need access to the CDRC resources through the DCS Organisation Group.
If you do not currently have access, please follow the instructions on the CDRC SystmOne Access webpage.
Once access has been enabled, the reports can be found in the relevant CDRC folders and the templates can be opened from within the patient record.
Clinical Reports
CDRC has developed a number of reports to identify patients who are eligible for a NHS Health Check. These reports have been sub-categorised into specific patient cohorts allowing you to target specific cohorts.
These reports are located in the CDRC > NHS Health Checks folder.
The CDRC > NHS Health Checks 2025 2026 folder contain reports that will support the tracking of your practices’ NHS Health Check performance
Patient Status Alerts
CDRC Patient Status Alerts are not activated automatically when an organisation joins the DCS Organisation Group. This allows each organisation to retain control over its local SystmOne configuration.
To request activation:
- Email contact-cdrc@healthinnovationnenc.org.uk
- Request activation of the CDRC Safeguarding Patient Status Alert.
- A member of the CDRC team will support the organisation through the activation process.
Once activated, the Safeguarding PSA appears to make relevant safeguarding information more visible when a record is accessed.

NHS Health Check Completion Template
The NHS Health Check Completion – CDRC Template supports clinicians in recording a complete NHS Health Check and ensuring all required information has been captured
How to Access
With the patient record open, use the search bar in the lower left-hand corner and search for: NHS Health Check Completion – CDRC
Alternatively, press F12, search for and select the template from the search results.
The buttons on the left-hand side of the template will direct you to the relevant area to input outstanding information.
Clicking on the NHS Health Check – Print Report and Complete Check will open a protocol that will inform you if the NHS Health Check has been completed, if any data is missing from the health check the Question box will ask if you would to complete this data, or to show missing data, or to continue anyway – not advised.
Clicking on the appropriate response will allow you to view any missing required data and to complete any required fields.
