There is a suite of resources available identify patients and conduct an NHS Health Check within CDRC Resources.
To access the below resources you will need to be a member of the DCS group on SystmOne. To do this, please follow the instructions on the CDRC SystmOne Access webpage.
NHS Health Check Reports
CDRC has developed a number of reports to identify patients who are eligible for a NHS Health Check. These reports have been sub-categorised into specific patient cohorts allowing you to target specific cohorts.
These reports are located in the CDRC > NHS Health Checks folder.

The CDRC > NHS Health Checks 2025 2026 folder contain reports that will support the tracking of your practices’ NHS Health Check performance

Healthy Heart Check – Health Innovation Project
As part of the wider Cardiovascular Disease Prevention Programme, Health Innovation North East and North Cumbria is leading a Healthy Heart Checks programme to support underserved communities across the region. In collaboration with the Health Innovation NENC, CDRC has developed reports specific for the Healthy Hearts Checks Programme.
These reports are located in the CDRC > NHS Health Checks folder. There are considerable reports located in this folder which may be of benefit to you. However the reports that are specific to this programme are highlighted below:

Patient Status Alerts
The following two Patient Status Alerts are avialable to support NHS Health Checks.
To activate these PSAs, you need to go to Setup > Data Output > Patient Status alerts and enable to following:

You will then need to contact CDRC to activate these alerts at your practice. Please send an email to contact-cdrc@healthinnovationnenc.org.uk and specify that you would like the NHS Health Check PSA activated.
NHS Health Check Completion – CDRC Template
The NHS Health Check Completion – CDRC Template will support you in completing

Clicking on the NHS Health Check – Print Report and Complete Check will open a protocol that will inform you if the NHS Health Check has been completed, if any data is missing from the health check the Question box will ask if you would to complete this data, or to show missing data, or to continue anyway – not advised.
Clicking on the appropriate response will allow you to view any missing required data and to complete any required fields.
