The following resources have been developed in collaboration with the North East and North Cumbria Palliative Care and End of Life Team by a multidisciplinary team of specialists and generalists. They are intended to support clinicians responsible for managing symptoms in patients with palliative and end-of-life care needs, regardless of diagnosis.
Accessing CDRC Resources on EMIS
To access the below resources you will need to activate a Sharing Agreement with CBC Health Federation Ltd. To do this, please follow the instructions on the CDRC EMIS Access webpage.
Guidance on the following CDRC resources on SystmOne
To support users of the resources listed below, and in conjunction with the release of the updated Deciding Right documentation, this document provides additional guidance on how to access, navigate, and utilise the Template and Kardex resources described below.:
CDRC EMIS Palliative and End of Life Care Resource Guide
Palliative Care and End of Life Care Planning Template
- Within the open consultation, select Run Template. This will open the template picker search tool. In the search bar, enter Palliative and End of Life Care Planning Template to locate and access the template:

- The template consists of 5 pages to be completed and a disclaimer page

- The Palliative Care page consists of 5 different sections to be completed, including:
- Palliative Care Information
- Consultation/ MDT review Notes
- Individuals Involved Information
- Communication Decisions
- Ethnicity/ Religion/ Housing/ Marital status/ Social family Information

- The Regional Forms page consists of all information relating to any relevant forms that need to be completed/ reviewed including:
- DNACPR
- Treatment Escalation Plan
- EHCP (Emergency Health Care Plan)
- Advance Statement
- Advance Statement to Refuse Treatment
- Special Patient Notes (NEAS)

- To ensure that the supporting documents are generated, please make sure all relevant boxes are selected before completing and saving the template and consultation

- The Care of the Dying Document pagewhich should be completed accordingly

- The Prescribing page which should be completed accordingly

- The Death Review page which should be completed accordingly

- Once all pages of the template have been completed select save template

- Once the template has been saved, the consultation must also be saved for the prompt to launch the supporting documents appears

Palliative Care Kardex
Once all supporting documents have been completed, processed and saved a prompt to launch the Kardex will appear

Depending on the patients latest eGFR result a prompt will appear to enable the correct Kardex form is completed

Prior to the Kardex launch, an automatic prompt will appear to select and pre-populate any allergy information into the form

Once the relevant form has launched complete it accordingly for the patient
There are five available forms; the patient’s condition and any potentially conflicting medications will determine which Kardex is launched
Actions to be aware of:
- As required medication – do not prescribe syringe drivers in anticipation or advance until the patient needs them.

- Syringe driver page – prescriber responsible to complete this

- The forms include several drop-down menus containing the required medications, ensure these are selected correctly

Once the form is completed, select File > Save As

The Document Properties box will appear, in the Document Title add Version 1 to the title, If you are editing an existing version, select Save As and ensure version control is maintained by labelling the updated Version 1 file as Version 2, once completed select OK

Select File > Close

Prescriber responsible for ensuring community nurses are aware it has been completed and arrangements made to take to patients’ home
Palliative Care and End of Life Care Documents
Designed by Jonathon Pickard, the updated Palliative Care and End of Life resources documented have been digitised into EMIS for user benefit. For further information regarding these documents, please visit: Deciding-Right-Relaunch.pdf
The supporting EOL documents consists of seven different documents that should be completed accordingly, these consist of:
- DNACPR
- Treatment Escalation Plan
- EHCP (Emergency Health Care Plan)
- Advance Statement
- Advance Statement to Refuse Treatment
- Special Patient Notes (NEAS)
- Care of The Dying Document
- Once the consultation has been saved, a prompt to launch the relevant form (e.g. DNACPR) will appear. Select Yes, and the form will open in mail merge for completion.


Once the form is completed, select File > Save As

The Document Properties box will appear, in the Document Title add Version 1 to the title, If you are editing an existing version, select Save As and ensure version control is maintained by labelling the updated Version 1 file as Version 2, once completed select OK

Select File > Close

Once the first form has been completed and saved the same prompt for the next form to be completed will launch, follow the same procedure for each document completed, for example EHCP, Care of Dying and Special Patients Notes (NEAS)
Currently there is not a prompt to launch the Treatment Escalation Plan, Advance Statement and Advance Statement to Refuse Treatment, therefore these documents will need to be populated separately
To do this, select Add, scroll down to Document, and choose Create Letter. This will open the New Patient Letter search option.

Using the magnifying glass button, search for the required form and select it to open. Complete and process the form, then save it following the same steps outlined above

This process can also be used if the prompts to launch forms do not appear
The individual completing the documents is responsible for arranging for them to be delivered to the patient’s home in a yellow envelope.
