CDRC’s Diabetes resources have been designed to make it easy and efficient for practices and PCNs to identify patients at high risk of Diabetes, instigate appropriate interventions and to manage existing Diabetic patients.
Accessing the Population Reporting Searches:
Until recently, CDRC’s EMIS Population Reports have been made available to access directly from the CDRC website. However, this has meant that there is no way of ensuring that they are being used appropriately and updated as necessary, representing a risk.
You can still access these reports directly from the CDRC team by emailing contact-cdrc@healthinnovationnenc.org.uk ; a member of the CDRC team will be able to provide access to the required reports and any ongoing support.
We endeavour this to be a short-term solution as CDRC are working on a comprehensive support suite, which in the long-term will benefit GP practices using these resources, by providing a more in depth & ongoing support system.
Population Reporting Guidance:
view CDRC’s focused Population Reporting Diabetes searches navigate to the CDRC Quality Diabetes IGR (2nd December 2024) folder and open the CDRC Quality Diabetes IGR Focused Reports folder.

The Reports within this folder are split into different sections for ease of use.
The Prevention Reports support the accuracy of your Diabetes and Non-diabetic hyperglycaemia (NDH) register by identifying patients who may require an appropriate disease code.

The Casefinding / Screening reports identify patients who require review and the addition of a disease code if deemed appropriate.

The Management reports will support the ongoing care of your Diabetic patients, including key care processes.

Specific GLP1 Reports are included to identify patients who require review or have a coded safety issue.

CDRC Diabetes Template
To access the CDRC Diabetes Template, you will need to have the CDRC Precision Resource Launcher enabled at your practice. Information on how to do this can be found by clicking on the link below:
Once enabled, you will be able to follow these steps:
- With the identified patient’s record retrieved, press F12 and select CDRC Precision Resource Launcher.
- Click on Long Term Condition Review Templates (QoF) > Diabetes, which will open the CDRC Diabetes Template
The Diabetes Page allows you view previous records and input key relevant information onto the patient record.
