The CDRC CVD Prevention and Management Resource Suite supports GP practices to systematically identify, assess and manage patients who may have established cardiovascular disease or be at increased risk of developing cardiovascular disease.
The resource suite combines:
- CDRC Reports for population-level case finding, clinical prioritisation and quality improvement.
- The CDRC Precision CVD Prevention Template for individual patient assessment, coding and management.
- A patient-level alert highlighting relevant CVD prevention information within the EMIS record.
- Clinical decision support covering cardiovascular risk, blood pressure, lipids, atrial fibrillation, chronic kidney disease, weight and glycaemic control
Together, these resources provide a structured pathway from population-level identification to individual patient review, clinical action and follow-up. The Population Reporting suite currently includes searches covering Atrial Fibrillation, Coronary Heart Disease, Heart Failure, Hypertension, PAD, Medication Optimisation and drugs prescribed without a corresponding diagnosis

Accessing the CDRC Reports
CDRC Population Reporting searches are provided directly by the CDRC team. This helps ensure practices receive the appropriate and most current version of the searches, together with any necessary implementation support.
To request access, please email contact-cdrc@healthinnovationnenc.org.uk and a member of the CDRC team will will provide information about access and available support.
This is a temporary arrangement while CDRC develops an enhanced EMIS support service offering improved resource access, regular updates and ongoing support for participating practices.

CDRC Cardiovascular Reports
The CDRC Cardiovascular reports support the identification and management of patients with diagnosed or potentially undiagnosed cardiovascular disease. The structure of this report folder is broken down into condition areas.
The Atrial Fibrillation reports support screening, case finding and the management of patients with recorded or possible atrial fibrillation. [cdrc.nhs.uk]
The reports may support practices to:
- Identify patients who may require assessment for AF
- Review patients with an existing AF diagnosis
- Identify potential coding gaps
- Prioritise patients who may require a structured AF review
- Support review of stroke and bleeding risk information
- Identify opportunities to review treatment
The Coronary Heart Disease reports include case-finding searches designed to support the identification and coding of patients with possible CHD. [cdrc.nhs.uk]
Suggested practice action
For each identified patient:
- Review the patient’s diagnoses and cardiovascular history.
- Review relevant correspondence, investigation results and procedures.
- Determine whether the clinical record supports a CHD diagnosis.
- Correct or add clinical coding where appropriate.
- Review cardiovascular prevention and medication management.
- Arrange further assessment where the diagnosis remains uncertain.
- Record the outcome and follow-up plan
The Heart Failure reports support case finding and management, including the identification of patients who may require review of their diagnosis, coding or current management.
The Hypertension reports support screening, case finding and management of patients with recorded or possible hypertension. The existing reporting guidance recommends reviewing identified patients, repeating blood pressure measurements, updating disease coding and optimising treatment where appropriate.
Use the dedicated blood-pressure page within the CVD Prevention Template to support structured assessment and recording.
The Medication Concordance reports support the identification of patients whose prescribing or medication record may indicate that a treatment review is required.
The Drugs to Diagnosis reports help identify patients receiving medication without an expected corresponding diagnosis in the clinical record. [cdrc.nhs.uk]
A report result may reflect:
- Missing or incomplete diagnosis coding
- Treatment initiated for another indication
- Historic medication that requires review
- Information recorded elsewhere in the clinical record
- A genuine need for further diagnostic assessment
The Specific Medications reports support safe prescribing by identifying patients who may have a medication-related safety risk or whose current treatment may require review to determine whether a safer alternative is appropriate.
The Peripheral Arterial Disease reports support case finding, coding review and safer prescribing for patients with recorded or potential peripheral arterial disease.
The reports identify patients who may have:
- Medication associated with peripheral arterial disease without a corresponding PAD diagnosis
- A PAD diagnosis code without the relevant QOF code
- A recorded PAD diagnosis where antiplatelet treatment may require consideration
The Stroke and Transient Ischaemic Attack reports support case finding, coding review and safer prescribing for patients with a recorded or potential history of stroke or transient ischaemic attack.
The reports identify patients who may have:
- A potential indicator of stroke or transient ischaemic attack without a corresponding QOF diagnosis code
- Vascular dementia codes without a recorded history of stroke or transient ischaemic attack
- A recorded stroke where the stroke type has not been classified
- Ischaemic or unclassified stroke where antiplatelet treatment may require consideration
CDRC Precision: CVD Prevention PSA
When a patient record is opened, the CDRC Precision: CVD Prevention alert will appear where relevant. Hover over the alert to view key cardiovascular prevention information, as shown below.
The alert can support both consultation-based care and planned patient record reviews. Review the information alongside the patient’s full clinical record before deciding whether further assessment or action is required.
CDRC Precision CVD Prevention Template
The CDRC Precision CVD Prevention Template supports structured assessment and management across multiple areas of cardiovascular prevention.
The template:
- Responds to coded information held in the patient record
