The CDRC has developed a number of reports to identify patients eligible for an NHS Health Check.
Accessing the NHS Health Check Population Reporting Searches
Until recently, CDRC’s EMIS Population Reports have been made available to access directly from the CDRC website. However, this has meant that there is no way of ensuring that they are being used appropriately and updated as necessary, representing a risk.
You can still access these reports directly from the CDRC team by emailing contact-cdrc@healthinnovationnenc.org.uk ; a member of the CDRC team will be able to provide access to the required reports and any ongoing support.
We endeavour this to be a short-term solution as CDRC are working on a comprehensive support suite, which in the long-term will benefit GP practices using these resources, by providing a more in depth & ongoing support system.
Guidance on the NHS Health Check Reports
To view CDRC’s focused Population Reporting CDRC NHS Health Check Eligible Patient reports navigate to the CDRC NHS Health Check Eligible Patients (17/07/25) folder and open the Eligible Patients Focused Reports folder.

Within the Eligible Patients Focused Reports folder, there will be reports to identify patients eligible for a NHS Health Check, broken down into sub-reports for those who who have not been invite in the last 5 years, last 1 year and those who have not been invited three times in the last year. These reports have been sub-categorised into specific patient cohorts allowing you to target these cohorts if you desire.

Healthy Heart Check Programme
As part of the wider Cardiovascular Disease Prevention Programme, Health Innovation North East and North Cumbria is leading a Healthy Heart Checks programme to support underserved communities across the region. In collaboration with the Health Innovation NENC, CDRC has developed reports specific for the Healthy Hearts Checks Programme.
These reports are accessed by importing the document above and are located in the Eligible Patients Focused Reports sub-folder.
The reports that are specific to this programme are:
- ? 3.42 IMD Quintile 1 or IMD Quintile 2
- ? 3.421 Not invited in the last 5 years
- ? 3.422 Not invited in the last 2 year
- ? 3.423 Not invited three times in the last year
